Sex is determined by autosomal gene expression.
Though you can't change your reproductive system because it's built while inside the womb, you can block male puberty and take female hormones and develop the rest of your body just like any other natal female.

Skin changes with hormones too, and trans women on HRT can get cellulite, just like any natal female.
Trans women are female, trans men are male. (Considering they already are on HRT)
They are just not natal females or natal males respectively, because they weren't born taking HRT, of course.
